How it works:
Following a physician’s referral or care plan, our team conducts a thorough initial assessment. Nurses are then assigned based on specialization and client needs. Each visit includes clinical documentation, ongoing health monitoring, and updates to care plans when necessary. Coordination with physicians, family, and other caregivers ensures continuity of care.
